Radiopharma Is Building at Record Speed. The Talent Pool Isn’t

The radiopharmaceutical industry has spent the past several years trying to solve its capacity problem. New isotope production systems are coming online. Manufacturing facilities are being built and expanded. CDMOs are adding hot cells and production suites. Pharmaceutical companies are assembling increasingly ambitious radiopharmaceutical pipelines, while hospitals and nuclear pharmacies prepare for the possibility that radiopharmaceutical therapy could eventually treat substantially larger patient populations.


It is an extraordinary buildout. But underneath it is another capacity problem that has received far less attention. The industry needs people to run all of it.


That may sound like an ordinary challenge for a fast-growing sector, but radiopharma is

 not an ordinary labor market. Many of the positions required to develop, manufacture, distribute and commercialize radioactive medicines demand experience that remains concentrated among a relatively small community of professionals. As more companies enter the market and established players expand simultaneously, they are increasingly competing for the same people.


“Radiopharma is a unique space from a recruitment perspective,” Millie Bone, Principal Associate at Mantell Associates, told Nuclio News. “The talent pool is highly specialised, relatively small, and extremely interconnected.”

Millie has spent five years as a headhunter and now focuses specifically on the radiopharmaceutical market, working across areas including CDMOs, biotechnology, isotope supply and production and nuclear imaging. She specializes in finance, HR and commercial appointments, while Mantell Associates uses dedicated recruitment verticals to cover quality, technical, operations, clinical and other specialized functions. 



From that vantage point, Millie is watching a labor market evolve alongside the industry itself. The challenge is no longer simply finding someone with pharmaceutical experience. Increasingly, companies want people who understand the peculiarities of radiopharma: its companies, technologies, regulatory environment, supply chains, commercialization challenges and unusually interconnected network of competitors and partners.


“Because of that, success in this market is not simply about having access to a database of candidates,” Millie said. “It is about genuinely understanding the ecosystem, knowing the companies, understanding where talent is moving, and having established relationships with the people operating within it.” 

That observation points to a larger issue facing nuclear medicine. The physical infrastructure supporting the industry may now be expanding faster than the experienced workforce available to operate it.


The Same Expansion Creating Opportunity Is Creating Competition

Radiopharma's talent challenge is partly a consequence of its success. A decade ago, the universe of companies aggressively building radiopharmaceutical businesses was considerably smaller. Today, specialized radiopharmaceutical developers are competing alongside major pharmaceutical companies, isotope producers, CDMOs, radiopharmacies, imaging companies and other organizations expanding into the sector.


Every new facility and program creates another layer of hiring demand. Manufacturing expansion requires operations and quality personnel. New clinical programs require clinical and regulatory teams. Companies approaching commercialization need market access, business development and commercial leadership. Isotope producers require technical and operational expertise. Health systems expanding theranostics programs need nuclear medicine physicians, technologists, pharmacists, physicists, nurses and radiation safety professionals.

The difficulty is that many of those organizations are drawing from overlapping pools of experience.


Millie sees those dynamics through the movement of people between companies. A significant part of her work involves tracking where organizations are gaining and losing talent, which companies are building particular functions and where potentially accessible candidates are located.  That makes recruiting in radiopharma surprisingly close to competitive intelligence.


When Millie works with organizations, she says the exercise goes beyond presenting candidates. It involved mapping how talent is moving around the businesses and across competitors. Her analysis includes talent flows and examining connections within organizations and the roles that are in demand. 


Those movements can reveal something important about the industry's development. Companies do not build capabilities simply by constructing facilities or acquiring technology; they build them by assembling teams of people who have already solved similar problems elsewhere. As a result, experienced talent is increasingly becoming a strategic asset.


A company that hires someone who helped scale another radiopharmaceutical operation is acquiring more than an employee. It is bringing in knowledge about processes, customers, competitors and the practical realities of operating in a specialized market. When several companies attempt to acquire that expertise at the same time, the value of the relatively small number of people who possess it naturally increases.


Why Traditional Recruiting Becomes More Difficult

This is also why simply posting a job may not solve the problem.


In larger industries, employers can often rely heavily on active candidates. Radiopharma's smaller labor pool changes the equation because many of the most attractive candidates are already employed and may have no intention of applying for another position.


“In a market like radiopharma, where many of the strongest professionals are already known within the industry, the difference comes down to access, relationships, timing and understanding,” Millie said. 

Mantell Associates consequently approaches much of its work as headhunting rather than conventional applicant recruitment. Before launching a search, Millie says the firm attempts to understand the company's strategy, growth plans, challenges, culture and competitors. Only then does it begin mapping relevant organizations and individuals, approaching people and assessing whether their motivations and experience fit the opportunity. 


The objective is deliberately narrow. Rather than sending dozens of resumes to a hiring manager, the firm attempts to produce a much smaller group of candidates who have already been evaluated against the company's needs.


That approach was put into practice through Mantell's work with Nucleus Radiopharma, where the firm supported searches that resulted in three senior appointments: Chief Business Officer, Chief Financial Officer and Vice President of Commercial Operations. For each search, Mantell developed a shortlist of approximately 15 candidates after mapping relevant companies and individuals. 


The value of that process, Millie argues, extended beyond the three people ultimately hired. The searches created a wider map of professionals who had been identified, assessed and engaged, giving Nucleus greater visibility into the talent surrounding its business and potentially relevant candidates for future growth. 


That is an important distinction because hiring in a small industry is rarely an isolated event. A company building a radiopharmaceutical platform today may need a commercial leader, a quality executive or an operations specialist six months from now. Understanding where those people sit before the position opens can become part of long-term workforce planning.


The Industry May Have to Widen the Talent Pool

There is, however, an obvious limitation to moving experienced radiopharma professionals from one company to another: it does not create any new talent.


Eventually, the industry has to expand the pool.


For some positions, that will mean looking beyond nuclear medicine. Mantell Associates operates across broader life sciences markets, and Millie says that allows the firm to identify candidates from adjacent areas such as biologics, cell and gene therapy and other specialized pharmaceutical environments when the experience is transferable. 


That could become one of the more important workforce strategies for radiopharma over the next several years. Not every job requires someone who has spent a decade working with radioactive medicines. Experienced pharmaceutical professionals can bring valuable expertise in areas such as finance, human resources, commercial operations, clinical development, quality systems and manufacturing. The challenge is knowing where that experience transfers cleanly and where the technical or regulatory peculiarities of nuclear medicine make direct sector experience essential.


Companies that insist on radiopharma experience for every position risk competing endlessly over the same small group of people. Companies that assume conventional pharmaceutical expertise can be dropped seamlessly into every radiopharmaceutical role face a different risk.

Finding the balance will matter as the industry scales.


It also means recruitment firms themselves are becoming more specialized. Mantell Associates describes its model as combining broader global life sciences reach with dedicated market verticals, allowing recruiters to develop deeper expertise within individual sectors while still drawing from adjacent talent pools. The firm has more than eight years of experience across CDMO, biotech, pharmaceutical, diagnostics and CRO markets. 


For Millie, that specialized knowledge is what separates the work from simply searching a candidate database.


“We are not trying to replace internal talent teams,” she said. “We operate as a specialist bolt-on resource, bringing additional market knowledge, relationships and reach when a business needs access to talent that may not be actively looking.” 

Talent Is Becoming Part of the Capacity Equation

The larger issue for radiopharma is that the workforce question can no longer be separated from the industry's infrastructure question. When companies announce a new isotope production facility, manufacturing plant or radiopharmacy, capacity is typically expressed in physical terms: square footage, hot cells, production lines, curies or doses. Yet none of that infrastructure represents usable capacity without the people required to operate it.


A hot cell without qualified personnel is equipment. A manufacturing suite without trained operators and quality staff is a room. A radiopharmaceutical pipeline without experienced clinical, regulatory and manufacturing teams is a collection of assets that still has to be translated into medicines.


The same problem is beginning to appear downstream. If radiopharmaceutical therapy continues moving into larger cancer populations, health systems will need more than treatment rooms. They will need the physicians, technologists, pharmacists, physicists, nurses and radiation safety personnel required to treat those patients. That is why the talent shortage deserves to be viewed alongside isotope supply and manufacturing capacity as one of the industry's emerging constraints.


Radiopharma has attracted extraordinary investment because companies believe radioactive medicines can become an increasingly important part of cancer care. The industry is responding by building the physical infrastructure needed to support that future. Building the human infrastructure may take longer.


People cannot be manufactured on demand. Experienced professionals accumulate knowledge through years of working inside the industry, and as Millie points out, many of the strongest people are already known, and already working somewhere.


“We know the market, we know the people within it, and we understand what motivates them,” she said. “That allows us to do more than identify candidates, it allows us to identify the right people for the right businesses.” 

For companies competing in radiopharma's next phase, knowing those people may become almost as important as knowing where the next supply of isotopes is coming from. Because the industry's next bottleneck may not be something that can be solved with another cyclotron, reactor or manufacturing facility. It may be finding enough experienced people to run them.
